In New Zealand a sub-contractor died after a storage tank he was working on exploded on 4 September.
The worker was welding guard rails on top of an empty 18,000 litre bitumen tank at Fulton Hogans Greymouth plant when it exploded killing him instantly. Another man also suffered minor injuries.
Debris was found hundreds of metres from the site of the explosion, which could be heard and felt more than a kilometre away.
Police are still trying to establish what caused the explosion, and The Department of Labour is currently investigating the incident.
